I understand that this form is my GSDBA Membership Agreement, but it is not my listing for the GSDBA Directory and/or website. To be included in the Directory and/or on the website, I understand that I will need to complete the Directory Sales Forms and/or the Internet Listing Form. If applicable, I certify that I have and will maintain my appropriate professional or occupational license(s). I acknowledge that my GSDBA membership dues are nonrefundable.
